Occupational Health Program Enrollment
Purpose: A fundamental responsibility of the Occupational Health Program (OHP) is to reduce the health and safety risks and hazards associated with animal contact. The OHP accomplishes this task by having appropriate personnel complete an animal contact risk questionnaire. The information from the questionnaire is connected with other campus hazard identification data and animal use materials. This information is used to determine required immunizations, use of personal protective equipment, hygiene practices, medical surveillance, and training. The act of completing the animal contact risk questionnaire enrolls that person into the occupational health program.

Policy: Enrollment in the University's Occupational Health Program is accomplished by completing an animal contact risk questionnaire. Enrollment in the Occupational Health Program is mandatory for the following groups of participants:
  • Faculty, staff, and students named in an approved animal care and use protocol for teaching, research, or outreach.
  • Any visiting scientist, investigator, faculty, or researcher who intends to spend more than 7 days (non-cumulative) in a UW-Madison facility and will work with live vertebrate animals during that time. Any visiting scientist, investigator, faculty, or researcher who intends to spend more than 30 days (non-cummulative) under these conditions must complete required Occupational Health and Safety training.
  • Research animal veterinarians, research animal pathologists, and their staffs. Certain office staff who have no animal contact may be excluded.
  • Animal care staff. This includes individuals who work or volunteer in animal facilities, and individuals who reside at certain UW-Madison farms but who are not faculty, staff, or students.
  • Animal Care and Use Committee members, including lay members & ex-officio.
  • Facility, maintenance, and safety personnel who provide service support to animal care facilities, including equipment and devices housed there.
  • Veterinary medical students.
  • Veterinary Medicine Teaching Hospital (VMTH) staff including: faculty with clinical duties, staff veterinarians and residents, veterinary technicians and barn personnel, reception, medical records and other VMTH office staff, pharmacists, pharmacy staff, and central supply staff, and VMTH employed facility and maintenance personnel.
